Introducción
El mérito de haber descrito por primera vez
una afección demencial, que más tarde se
conocería como enfermedad de Alzheimer,
corresponde al fisiatra y neuropatólogo ale-
mán Dr. Alois Alzheimer. “La enfermedad
de Alzheimer (EA) es una forma agresiva de
demencia que se manifiesta en déficits de
memoria, lenguaje y comportamiento” (Al-
zheimer’s Association, 2010).
Según las estimaciones de la Organización
Mundial de la Salud (OMS), la prevalencia
general proyectada en la población mundial
se cuadruplicará en las próximas décadas,
alcanzando los 114 millones de pacientes
para 2050, “Además de tener un gran im-
pacto social, esto claramente conduciría a
una mayor carga económica para los sis-
temas de salud en todo el mundo” (Chiang
& Koo, 2014). No hay opciones efectivas
disponibles en la actualidad para la preven-
ción y el tratamiento, a pesar de todos los
informes científicos existentes.
Se sabe que la enfermedad de Alzheimer
progresa gradualmente y puede durar dé-
cadas. En este sentido, se tiene que existen
tres etapas principales de la enfermedad,
cada una con sus propios desafíos y sínto-
mas. Al identificar la etapa actual, los médi-
cos pueden predecir qué síntomas se pue-
den esperar en el futuro y posibles cursos
de tratamiento.
Cada caso de EA se presenta con un con-
junto único de síntomas, que varían en se-
veridad. La herencia de ciertos genes es un
factor de riesgo para la EA, con casos tanto
familiares como esporádicos. “En la EA es-
porádica, que es la forma más común, exis-
te un vínculo con el alelo de la apolipopro-
teína 4 (APOE4), siendo el riesgo mayor en
situaciones homocigóticas” (Chou, 2014).
Los factores ambientales, los factores vas-
culares y los factores psíquicos contribuyen
al desarrollo de la enfermedad. Actualmen-
te, no hay medicamentos disponibles para
detener la progresión de la neurodegenera-

ción por lo que la naturaleza del tratamiento
es sintomática.
Los inhibidores de la colinesterasa (CI) que
promueven la neurotransmisión colinérgi-
ca se usan en casos leves a moderados.
La memantina, un antagonista del receptor
de N-metil-D-aspartato (NMDA), se usa en
casos moderados a graves para prevenir
la excitotoxicidad, y los antipsicóticos y los
antidepresivos se usan en el tratamiento de
los síntomas neuropsiquiátricos (Ballard &
Corbett, 2010).
Sin embargo, en este momento, no exis-
te una forma comprobada de prevenir el
Alzheimer. La investigación sobre estrate-
gias de prevención continúa y se desarro-
lla día a día. La evidencia más sólida has-
ta el momento sugiere que es posible que
pueda “reducir su riesgo de enfermedad
de Alzheimer al disminuir el riesgo de en-
fermedad cardíaca. Muchos de los mismos
factores que tienden a aumentar su riesgo
de enfermedad cardíaca también pueden
aumentar su riesgo de enfermedad de Al-
zheimer y demencia vascular” (Alzheimer’s
Association, 2010).
Los factores importantes que pueden estar
involucrados incluyen presión arterial alta,
colesterol alto en sangre, exceso de peso
y diabetes. La enfermedad de Alzheimer es
compleja, y es poco probable que un fár-
maco u otra intervención puedan conducir
con éxito en su tratamiento adecuado. Los
enfoques actuales se centran en ayudar a
las personas a mantener la función mental,
controlar los síntomas conductuales y retar-
dar o retrasar los síntomas de la enferme-
dad.
Los investigadores esperan desarrollar te-
rapias dirigidas a genes moleculares, por lo
que se cree que “el futuro del tratamiento
radica en atacar las placas neuríticas (NP)
y los ovillos neurofibrilares (NFT), que tienen
el potencial de retrasar la neurodegene-
ción” (Chiang & Koo, 2014). Este artículo de
